HEELKUNDIGE INGREPEN

1. Chirurgische aandoeningen bij de neonaat
Welke pathologieën vereisen chirurgie?
 congenitale darmafwijkingen
 anale atresie
 spina bifida
 omphalocoele
 gastrochisis
 slokdarmafwijkingen
 cardiochirurgie
 urologische aandoeningen
 hernia diafragmatica

2. Pre-operatieve verpleegkundige aandachtspunten
2.1 Nuchter houden
Het doel van nuchter houden is om de kans op aspiratie van maaginhoud te
verkleinen.

Hoe kinderen nuchter houden?
 voeding
Kunstvoeding mogen kinderen 6u voor de operatie niet meer en borstvoeding
4u voor de operatie.
 medicatie
Alle enterale medicatie (= toediening via maag-darmstelsel) zetten we stop en
vervangen we door IV-medicatie.

Kinderen met reflux gaan we al 12u voor de operatie nuchter houden. Bij darmartresie
moet men een maagsuctie doen.

Gevaren van nuchter zijn:
 dehydratatie
 hypoglycemie
 discomfort
 verhoogde maagzuurproductie

2.2 Infuustherapie
Het doel van infuustherapie is de vochtbalans op peil houden.

Aandachtspunten:
 rekenning houden met operatiegebied
 bestaand infuus controleren
 bij voorkeur centraal infuus met meerdere lumens of 2 infusen voorzien
 infuussnelheid aanpassen

Pre-operatief gaan we ook een bloedafname gaan uitvoeren voor een kruisproef zodat
het bloed standby ligt en zo nodig toegediend kan worden.

2.3 Pre-operatieve fiche
Het doel van de pre-operatieve fiche is dat alle belangrijke info direct bij de hand is.
De fiche wordt steeds ingevuld door de arts.
