BPD criteria correct answers-period of mania or hypomania - judged based on duration and intensity -mania requires a 7 day episode where hypomania requires a 4 day episode -BPD II differs from BPD I in that it requires one MDE Case 3.1- Emotionally Disturbed- Donald - man comes in to the ER with extreme grandiose dellusions, says he is the "New Jesus", requires restraint and heavy medication, says he received a message from God and no longer sleeps, released twice before with schizophrenic affective disorder, currently displaying rapid speech, leg bouncing, seemed very agitated at misunderstandings correct answers- Diagnosis BPD-1, current manic episode, severe, with mood congruent psychotic features -Why?-Presented to the ER with irritable mood, grandiosity, diminished need for sleep, pressured speech, racing thought, agitation, sexual inappropriate behavior, blisters from insistent walking, dehydration, all indicative of a MANIC episode -He had maintained a job normally until this recent episode which is high unlikely of someone with schizophrenia, his psychotic features were more polarized around the manic episode at hand, african americans are diagnosed with schizophrenia more frequently for unknown reasons Case 3.2- Nancy Ingram- Cycles of Depression husband reports "dark periods", tearfulness w/extreme agitation/anger and no sleep. He brought her in because she became obsessed with a stock market blog she created that was not only out of character but against company policy as she worked for a investment bank, she avoided other responsibilities to maintain the site insisting it would make them rich. She lost her father to suicide causing an episode when she was 18, she was diagnosed with depression and has recurrent periods of dysphoria, insomnia, rapid speech and hyper altertedness, her speech during the interview was rapid, pressured and difficult to interpret, her eyes were glazed over, she insisted it was a misunderstanding correct answersDiagnosis- BPD, current manic episode, mixed features/rapid cycling -depression was not the correct diagnosis, her rapid speech is very uncharacteristic of depressed patients as well as her grandiose delusions -elevated use of alch and clonazepam common
