PMHNP Exam June 2022

1. NP explained all the risk and benefits of medication to the patient. Then immediately
submitted to the pharmacy for refill. What violation? – Informed consent (did not ask
the patient informed consent to send med to the pharmacy)

2. Patient on carbamazepine also take another medication, cannot remember starts with
D. – I answered increase medication D because Carbamazepine is an inducer.

3. Patient taking Lithium and Indomethacin combined. What signs and symptoms will
experience? - tremors, palpitations, weakness (lithium toxicity signs)

4. Recovery model for substance abuse, using non-linear -? not sure. I answered make
self-improvement for oneself. Other choices: utilize community resources? The patient
make decision

5. Use of Recovery model – another question on recovery model. That threw me off. – I
answered the patient will work towards compliance versus non-compliance on meds.

6. 5 yr old sodomize by brother – do not leave 5yr old with the brother and call CPS

7. One of the elements of cognitive therapy - make a consistent schedule with the patient

8. Borderline Personality patient became aggressive and rude to the PMHNP – therapeutic
communication: “This behavior is unacceptable, and we have to work out how to
communicate with each other”.

9. The question did not say neuronal loss or cell death. The questions say retraction of
neurons during child behavior modification. Choices: Apoptosis, Pruning, Potentiating

10. Compared to adults, school-age children – I answered something with neuronal
pathways in the answer choices. Other choices: Increase glomerular filtration rate,
increased motility/decreased absorption

11. Older adult with MMSE of 26, with loss of appetite, decreased concentration in the past
2 weeks, “tried” to answer questions from the PMHNP.
Answer: Depression (this patient showing signs of pseudo-dementia which is commonly
experienced in older adults with depression).
Other choices: Alzheimer’s (this is progressive and not gonna happen within 2 weeks),
HIV related dementia, fronto-temporal dementia

12. Patient came to the clinic admits on abusing using IV drugs – test for HIV

13. Severe level on MMSE – choices: 17, 20, 25, 30 – I answered 17. Poorly written question
