When factor Xa is blocked, it decreases the amount of an enzyme called thrombin in your body. Thrombin is a substance in your blood that's needed to form clots. When thrombin is decreased, this prevents a clot from forming. Rivaroxaban has a dual mode of elimination; two-thirds of the drug undergoes metabolic degradation in the liver, half of which is excreted via the kidneys and half via the hepatobiliary route. One-third of the dose is eliminated as unchanged drug in the urine Pharmacokinetics 06/15/2021 6 Rivaroxaban “Xarelto” Dosage Scheduli ng Route Length of therapy • 10 mg • 15 mg • 20 mg 15 mg orally every 12 hours for 21 days with food, THEN 20 mg orally once/day for 6 months The tablets are only available in an oral route and taken with food. If you are unable to swallow the tablets whole: • tablets may be crushed and mixed with a small amount of applesauce, followed by food. • If you are using a nasogastric tube or gastric feeding tube: tablets may be crushed and suspended in 50 milliliters (mL) of water before giving it via the tube, followed by enteral feeding. • 15 mg BID, 12 hours apart, for 21 days • After 21 days, use 20 mg once per day • After 90 days, the client will follow up in the thrombosis clinic to evaluate if they meet criteria for discontinuing the medication At least 6 months of standard anticoagulant treatment 06/15/2021 7 Rivaroxaban is a selective factor Xa inhibitor administered once or twice daily.
