NURS 6521 Pharmacotherapeutics for advanced practices_Drug Table & Drug Category Warnings GI – peptic ulcers - antibiotics Clarithromycin (Biaxin) Amoxicillin Bismuth Tetracycline Recommend ed for all pts with symptomatic gastric or duodenal ulcers and confirmed H pylori infection H pylori infxn Single use contraindicated – ineffective and increased risk of resistance Avoid in pregnant women and children Inhibits protein synthesis to suppress H pylori growth Disrupts cell wall Disrupts cell wall to cause lysis and death Inhibits urase activity Prevents H pylori from adhering to gastric surface Inhibits protein synthesis Highly effective but resistant in 20% H pylori is highly sensitive to amoxicillin, resistance is low 3% Enhanced when gastric acidity is reduced (ie use with omeprazole) Harmless black coloration to tongue and stool Highly effective with less than 1% resistance Diarrhea, nausea, distortion of taste Diarrhea Harmless black coloration to tongue and stool Long term use – risk for neurological injury Can stain developing teeth Recommend using 2 or 3 antibiotics x 10- 14d to avoid resistance and PPI or H-2- Receptor Antagonis
