THE FOUR PHASES AND KEY CHARACTERISTICS OF SIT TO STAND




Phase Key characteristic
I. Flexion - From the start of the movement to seat-off (when the buttocks leave the base of support
momentum and just the feet are in contact with the ground)
- Horizontal linear momentum is generated to move the body mass forwards over the feet
- The trunk and pelvis rotate anteriorly as the hips flex
- The head, spine and pelvis are usually considered to behave as one segment, with the
spine erect as the whole segment rotates forwards
II. - From seat-off to maximum dorsiflexion (the knees move forwards over fixed feet)
Momentum - Upper body flexion momentum is transferred into extension as body displacement shifts
transfer from the anterior direction of phase I to a forwards and upwards displacement in phase II
III. - From maximum dorsiflexion to full hip extension
Extension - The body is brought into a fully upright posture
IV. - From the end of hip extension until all motion associated with rising has stopped
Stabilisation - The completion of this phase is difficult to determine because of the observable normal
postural sway associated with quiet stance

COMMONLY OBSERVED DIFFICULTIES IN STS POST STROKE?

- Plaatsing van de voeten achter de knieën
o Belangrijk voor balans
o COM moet over de BOS geplaatst worden
- MAX DF zodat de knie naar voor verplaatst
o COM dichter bij de BOS
o Zonder dit zal de COM achter de BOS blijven en zal de Pt
naar achter vallen
o PT doet dit niet en duwt de knie tegen de tafel als
compensatie

- Voetpositie is niet goed
o Re voet te veel naar voor
- Overcompensatie van de Li arm, hij duwt zich hiermee te hard op
- Geen weightbearing op het recther been (ook re arm hangt erbij)
- Feedback
o Manual: sensorische info geven
o Visual: spiegel -> houding en aandacht brengen naar re
kant

De snelheid van deze sit to stand is veel te snel, hij heeft een veilige omgeving nodig om
deze beweging gecontroleerder en veiliger uit te voeren




Pt gaat veel te snel zitten en valt van te ver in de stoel
Welke aanpak ga je gebruiken om dit aan te pakken
- Visual feedback: plaats iets op de grond zodat de Pt kan zien waar hij zijn voeten
eerst moet zetten voordat hij gaat zitten (Pt heeft een cognitieve beperking)
- Eerste taak dat hij dus moet doen is zich correct voor de stoel plaatsen en dan
pas gaan zitten
