HC Fysiologie van het hart en basis ECG ZSO 13

In normaal patroon trekken eerst atria samen, erna ventrikels -> hartcontracties worden
gecoördineerd door geleidingsysteem -> net werk v gespecialiseerde hartspierncellen die
elektrische impulsen opwekken en geleiden.

Netwerk v geleidingsysteem bestaat uit:
Pacemakercellen / nodale cellen / gangmakercellen / prikkelvormige cellen
- Genereren prikkels en geleiden deze naar alle hartspiercellen
- Zelf geen contractiële eigenschappen
- Vormen netwerk op 2 plaatsen

o SA- knoop (sino-atriale / sinusknoop)
 Klompje gespecialiseerde cellen in wand RA, nabij opening VCS (vena cava superior)
 Genereren reguliere impulsen omdat elektrisch instabiel zijn -> hierdoor regelmatig
depolariseren (ontladen) ong 60-80 x / min -> erna repolarisatie
 Bepaalt hartslag, doordat SA-knoop sneller ontlaadt dan andere delen hart
-> primaire pacemaker (activering sinuskop -> atriumcontractie)


o AV-knoop (atrioventriculaire knoop)
 Klompje neuromusculaire cellen in want atriumseptum nabij AV kleppen
(atrioventiculaire … )
 Doorgeven elektrische signalen atria -> ventrikels (vertraging 0,1-0,2 sec)
 Door vertraging kunnen atria contractie eerst afronden voor ventrikels ermaa
beginnen
 Secundaire pacemaker -> neemt rol SA-knoop over indien problemen
 Intrinsieke snelheid trager (40-60 slagen / min) -> 2e snelste
 Gaat over in geleidende cellen -> bundel v His / atrioventriculaire bundel -> loopt in
interventriculair septum -> bundeltakken splitsen
 RBR (rechterbundeltak)
 LBT (linkerbundeltak)
-> netwerk v Purkinjevezels ontspringen -> dringen door in
ventrikkelwand in -> hier lijken cellen meer op gewone
myocardcellen
 Bepalen ritme contractie, depolariseren (+ worden binnenkant cel) spontaan en
genereren op regelmatige tijdstippen AP (actiepotentiaal)


Wanneer in nodale / pacemakercel AP gegenereerd wordt (70-80 actiepotentialen / min
want hartslag 70-80 slagen / min) -> geleidingsysteem voortgeleid -> bereikt alle
hartspiercellen -> veroorzaakt gecoördineerde contractie

Prikkels zullen eerst apex gaan stimuleren en later pas basis van het hart -> apex eerst laten
samentrekken omdat samentrekking v punt doet bloed naar boven duwen -> bovenste
spiercellen samentrekken -> bloed naar truncus pulmonalis en aorta
