This includes: ○ Physicians ○ Nurses ○ Occupational therapists ○ Physiotherapists ○ Speech-language pathologist ○ Social workers ○ Clinical nutritionists/dietitians ■ Pharmacists should be included to promote patient safety, medication reconciliation, patient/caregiver/family and team education about medication(side effects, adverse effects, drug interactions) and should be included for discussion about adherence and discharge planning ■ Additional members can include discharge planners, case managers, neuropsychologist, palliative care specialist, recreational and vocational therapists, spiritual care providers, peer supporters, and stroke recovery group liaisons ● Patient assessment should be within 48 hours of admission to facility and care/management plan should be created ○ Clinicians should use industry standard and valid assessment tools to evaluate stroke related impairments ○ Assessments should include: ■ dysphagia assessment ■ Mood and cogn
