Summary Changing the Culture in Medical Education to Teach Patient Safety
In 1999, the Institute of Medicine reported that 44,000 to 98,000 patient deaths were the result of preventable medical errors. This makes people try to find who is at fault in order to correct the error, but the Institute of Medicine had a somewhat different approach, their proposed solution involved both system errors and individual error in order to take corrective action. They concluded that, in order to correct this issue, a change in culture was required.
