1. Continuity of Care: Interventions Promoting Independence Proper organization of various community groups working together can help the continuity of care and promote independence for the client; such as: family, civic organizations, employment bureaus, etc. Community health nurses are essential for maintaining client health and safety throughout the transition from the hospital to home setting and furthering client education regarding their selfcare.
