1. Continuity of Care: Interventions Promoting Independence  Proper organization of various community groups working together can help the continuity of care and promote independence for the client; such as: family, civic organizations, employment bureaus, etc.  Community health nurses are essential for maintaining client health and safety throughout the transition from the hospital to home setting and furthering client education regarding their selfcare.  Discharge planning should always begin on admission to plan client needs after leaving the hospital; this should include the client, family, nurse, provider, case manager, and other important members of the health care team. 2.
