Exam (elaborations) NR 506 Week 1 TD Please discuss the four spheres of political action in nursing. In addition, please develop a brief argument sharing how these spheres are interconnected and overlapping by applying an example from your practice. What are some ethical considerations here? The nursing profession was not meant to exist as a task oriented job to only provide basic patient care. Nightingale had much higher aspirations in mind and I do believe she would be proud of the profession overall today. In today’s world, nurses are responsible for promotion, advocacy, protection and prevention of disease and injury not only for the patient but also for family members, the community, and other populations. Perhaps this is the reason nurses have decided to become more politically involved, so they can influence and effect change so that the care they provide can be improved upon. Per our textbook, the four spheres that nurses engage in are: the workplace or the workforce, the government, the community, and professional organizations or interest groups (Mason et al., 2016). The purpose of these spheres is to assist people in the comprehension of the policies. Each sphere has it’s own purpose or function but the spheres can also overlap, creating the capability of influencing others spheres as well (Mason et al., 2016). Nurses have the ability to impact policy making and in doing so can help create the change that is needed to improve healthcare as a whole. According to the ANA (2013), registered nurses have become looked upon as leaders in revamping healthcare and policy issues in order to meet the charge for change within wellness, prevention, and primary care services while also increasing the quality of care and managing the costs of it all. I do not have a personal example of how these spheres can be applied to practice but I was able to find an article with a great example. The article by McCracken (2010), provides an example of a group of nurse practitioners from Kentucky that noticed a significant amount of children, those with and without any sort of private insurance, had problems when it came to healthcare access. These nurse practitioners banded together and formed school-based health clinics for these children. A snag came when Medicaid would only pay them if a physician were onsite. Since this was near impossible they began working with state committees, state legislators, and a sponsor to prove nurse practitioners could provide quality care without a physician being present. These nurse practitioners were able to effect change and a bill was passed allowing nurse practitioners to practice without a physician being present. All four spheres are represented at some point in this example with overlapping in government, workforce, and community. Journal Of Gerontological Nursing, 36(3), 15-17. doi:10.3928/--05 SUMMARY: Dr. Holloway and class, During our first week we learned many concepts and policies related to healthcare policy. Healthcare policy is so important to medicine and patient care, something I feel after these next eight weeks will be even more evident. As nurses we must be flexible and ready for change, the same applies to healthcare policies. These policies must change in order to keep up with the needs of medicine in general. However, nurse must realize that it takes a team to make sure these changing policies are changing for the good of the patient, not for the benefit of government officials or for the benefit of our own emotions. This lesson discussed all the strong nurses in history that have stood up for change and advocated for the health of patients. These nurses are also the ones who discovered change was better made in groups. We are fortunate to reap some of the benefits from their hard work today. The lesson also discussed ethics in healthcare policy. The four ethical principles include beneficence, nonmaleficence, autonomy, and justice. As nurses, we will deal with ethical dilemmas often. We must follow policies in place and always act with out patients best interest in mind. This study source was downloaded by from CourseH on :0
