In dit document zal ik uitleg geven over hoe ik deze opdracht heb aangepakt. In overleg met mijn stagebegeleider heb ik een cliënt gekozen waarvoor ik dit ondersteuningsplan heb gemaakt. Het koppelen van de theorie met de praktijk was nog best lastig. Hierbij hebben mijn stagebegeleider en andere collega’s mij geholpen. Voor dit ondersteuningsplan, zijn bestaande OSP (cliëntdossiers) gebruikt. Deze zijn gebruikt om achtergrondinformatie van de cliënt te achterhalen. Daarnaast ben ik het gesprek aangegaan met de persoonlijke begeleiders, woonbegeleiders, gedragsdeskundigen en de cliënt zelf. Op deze manier heb ik de juiste informatie kunnen verzamelen en heb ik een onderwerp gekozen voor zijn ondersteuningsplan. De cliënt woont al meerdere jaren op de instelling en heeft een dossier van meer dan 40 pagina’s. Om het overzichtelijk te houden heb ik zelf een ondersteuningsplan gemaakt met hierin één onderwerp dat uitgewerkt wordt. Tot slot zal ik vanwege privacy en beschermingsredenen de cliënt niet volledig bij zijn naam noemen.
