Discuss the underlying pathophysiological mechanisms of your assigned disease process. Which
clinical manifestations observed in Wilbur’s case may be explained by the pathophysiological
mechanisms?
My assigned disease processes this week is allergies. A hypersensitive reaction is an abnormal
immune response to an antigen that leads to disease or harm in the individual (Rogers, 2023).
According to Mittal and Bhatt (2023), the fundamental biological processes behind allergic
reactions involve the activation of specific immune cells, including mast cells, basophils, and
eosinophils. These reactions are primarily mediated by Immunoglobulin E (IgE), an antibody
that attaches to allergens and stimulates the release of various inflammatory substances. These
substances include histamine, leukotrienes, and cytokines, which play a significant role in
producing allergic symptoms. The release of these mediators leads to inflammation, widening of
blood vessels, increased permeability of blood vessels, and contraction of smooth muscles, all of
which contribute to the physical manifestations of allergic responses. Allergies occur when the
immune system overreacts to harmless substances (allergens), triggering an inflammatory
response. This typically involves the release of histamine and other chemicals, leading to
symptoms like itching, swelling, and mucus production (Mittal & Bhatt, 2023).
However, Wilbur’s symptoms a long-standing purple rash, white coating on the tongue, and
frequent illnesses don’t strongly align with a typical allergic reaction. A purple rash that persists
for months, along with immune system problems (getting sick often) and oral changes (white
coating on the tongue), may suggest an issue affecting Wilbur’s immune system rather than a
simple allergic response.
Analyze Wilbur’s clinical manifestations as they relate to your assigned disease process. Do
these findings support your assigned disease process? Why or why not?
Wilbur’s clinical manifestations, including a persistent, non-itchy purple rash, a white coating on
the tongue, and frequent illnesses, are more indicative of an underlying immunocompromised
state than allergic reactions. Oral candidiasis, characterized by a white coating on the tongue, is
commonly associated with immune suppression resulting from factors such as extremes of age,
immunocompromising diseases like HIV/AIDS, and chronic systemic steroid and antibiotic use
(Talapko, 2021). Furthermore, studies have demonstrated that immunosuppressed individuals
