PART 1 — PHARMACOLOGY REVIEW



1.1 PHARMACOKINETICS
Definition: The study of how the body affects a drug—what the body does to the drug.
Pharmacokinetics encompasses four processes: ADME (Absorption, Distribution,
Metabolism, Excretion).


Absorption

• Definition: Movement of a drug from its site of administration into the
bloodstream.
• Factors Affecting Absorption:
o Route of administration (IV = fastest, oral = slowest)
o Blood flow to the absorption site
o Drug formulation (immediate-release vs. sustained-release)
o Presence of food in the stomach
o Gastric pH and motility
o First-pass effect (hepatic metabolism before systemic circulation)


Distribution

• Definition: Transport of a drug from the bloodstream to tissues and organs.
• Factors Affecting Distribution:
o Protein binding: Drugs bound to plasma proteins (primarily albumin) are
inactive. Only free (unbound) drug exerts therapeutic effect.

, o Blood flow: Highly perfused organs (brain, heart, liver, kidneys) receive
drugs more rapidly.
o Blood-brain barrier: Lipid-soluble drugs cross more easily.
o Placental barrier: Many drugs cross and can affect the fetus.

Metabolism (Biotransformation)

• Definition: Enzymatic conversion of a drug into a more water-soluble compound
for excretion.
• Primary Site: Liver (hepatic metabolism)
• Key Enzyme System: Cytochrome P-450 (CYP450) system
• Important Concepts:
o First-pass effect: Oral drugs are metabolized in the liver before reaching
systemic circulation, reducing bioavailability.
o Prodrugs: Inactive until metabolized into active form (e.g., enalapril →
enalaprilat).
o Enzyme inducers: Increase drug metabolism (e.g., phenytoin, rifampin,
carbamazepine, phenobarbital, St. John's wort). Result: decreased drug
effect.
o Enzyme inhibitors: Decrease drug metabolism (e.g., ketoconazole,
erythromycin, cimetidine, grapefruit juice). Result: increased drug effect
and toxicity risk.


Excretion

• Definition: Elimination of drugs from the body.
• Primary Route: Kidneys (renal excretion)
• Other Routes: Bile, lungs, sweat, saliva, breast milk
• Important Concepts:
o Creatinine clearance (CrCl) and glomerular filtration rate (GFR) are
used to assess renal function.
o Patients with renal impairment require dose adjustment for renally
eliminated drugs.
