Sloan Glen is a 10-year-old male brought to the clinic by his mother because of
declining school performance over the past six months. His mother reports
that Sloan previously performed well academically, but his grades have

have also noticed that he has become less attentive in class and sometimes
needs instructions repeated.
Sloan reports that he has difficulty concentrating during lessons and when
completing homework. He sometimes forgets assignments and loses track of
instructions. His mother reports that he frequently becomes frustrated when
doing schoolwork and takes longer than usual to complete assignments.
There has been no recent change in school or major family disruption. His
mother reports that he spends several hours each evening using electronic
devices and often goes to bed late. He usually sleeps approximately 7 hours per
night and occasionally feels tired during the school day.
Sloan denies persistent headaches, vision loss, hearing problems, seizures,
dizziness, or loss of consciousness. He denies feeling persistently sad or
hopeless. His mother has not noticed significant behavioral problems at home.
He continues to enjoy playing with friends and participating in sports.
