### History of Present Illness (HPI)
Cynthia is a 56-year-old obese female with a PMH of hypothyroidism
who presents for a blood pressure recheck after attending a workplace
health fair 3 weeks ago. Her blood pressure reading at the health fair
ranged from **132/74** to **156/94** mmHg depending on the case

, version . The nurse advised her to follow up with primary care within 4
weeks for re-evaluation.


She recently moved to the area approximately 5 months ago and has
not yet established care with a PCP. She has **not been monitoring her
blood pressure at home**.


**Associated Symptoms:**
- Reports occasional dull headaches, typically at the back of the head in
the evenings, resolving with rest
- Occasional mild dyspnea when climbing 2+ flights of stairs (new onset
over past 3-4 months)
- Fatigue that has gradually worsened over the past 2 months -
described as low-energy state, worse in afternoons
- Reports feeling "tired" easily; cold intolerance (consistent with
hypothyroidism)


**Denies:** Chest pain, palpitations, shortness of breath at rest, visual
changes, syncope, leg swelling, polyuria, polydipsia, polyphagia,
numbness, tingling, fever, or night sweats
