History of Present Illness (HPI)
Justin Johnson is a 19-year-old male who presents with his mother due to
problems at school. He was expelled from college two months ago for
breaking into a building on campus and shouting that the dean wanted to
steal his intellectual property.
, His mother reports that his behavior began changing about six months ago,
after he stopped taking his medication (methylphenidate) for Attention-
Deficit Hyperactivity Disorder (ADHD). Since then, he has stopped attending
classes and begun smoking more marijuana. He has become detached from
social interaction and is “obsessed” with research, specifically regarding
academic plagiarism.
Over the past two months at home, his mother has observed
manifestations of paranoid ideation and ritualistic behaviors. He has
neglected his personal hygiene, sleeps during the day, and avoids activities
he once enjoyed.
Past Medical and Psychiatric History
Asthma: History of childhood asthma; last episode at age 10;
previously used an albuterol inhaler.
ADHD: History of Attention-Deficit Hyperactivity Disorder.
Previous Psychiatric Episode: At age 13, he had a brief episode of
fear of leaving the house and talking to himself, which was attributed
to his ADHD medication at the time. His behavior returned to normal
after the medication was changed.
Social History
Occupation/Student Status: Male student recently expelled from
college.
Substance Use: Reports using marijuana.
