Introduction

Policy implementation is a critical stage of the public policy cycle because it transforms
government decisions into programmes, projects, and public services that address
societal needs and improve citizens' quality of life. However, even well-designed
policies may fail if implementation is poorly coordinated, inadequately resourced, or
resisted by those responsible for execution. In South Africa, policy implementation has
often been criticised for relying predominantly on hierarchical or top-down
administrative structures that separate policy formulation from implementation, limiting
the participation of frontline public servants and communities and contributing to poor
service delivery, resource inefficiencies, and reduced public trust in government
institutions (Cloete & De Coning, 2016). Public policy scholars have therefore debated
the merits of the top-down approach, which emphasises centralised decision-making
and hierarchical control, and the bottom-up approach, which promotes the active
involvement of frontline officials and stakeholders in adapting policies to local
conditions (Pressman & Wildavsky, 1973; Lipsky, 1980; Sabatier, 1986). This
discussion critically compares the advantages and disadvantages of these two policy
implementation approaches with reference to the South African Department of Health,
evaluates their respective outcomes, and illustrates their practical application through
examples such as the National Health Insurance (NHI), HIV/AIDS programmes, the
COVID-19 response, Primary Health Care (PHC), Community Health Workers, and
Clinic Committees.

Theoretical Perspectives on Policy Implementation

Policy implementation refers to the process through which governmental decisions are
translated into operational programmes, administrative actions, and public services. It
represents the stage where legislation, policy frameworks, and strategic plans are
transformed into measurable outcomes for society. According to Pressman and
Wildavsky (1973), implementation involves numerous actors operating across
different organisational levels, making coordination, communication, and
accountability fundamental determinants of policy success.

The implementation literature has traditionally been dominated by two contrasting
schools of thought. The first is the top-down approach, which argues that
implementation should be directed by senior political leaders and bureaucrats through

clearly defined objectives, standardised procedures, and hierarchical accountability.
The second is the bottom-up approach, which emphasises the discretion exercised
by frontline officials and the importance of stakeholder participation in adapting policies
to local realities (Sabatier, 1986).

Lipsky's (1980) theory of Street-Level Bureaucracy significantly influenced the
development of bottom-up implementation by demonstrating that frontline public
servants, such as nurses, teachers, police officers, and social workers, inevitably
modify policies during implementation because they interact directly with citizens and
must respond to complex local circumstances. Consequently, policy implementation
cannot simply be viewed as mechanical compliance with directives issued by central
government but rather as a dynamic process involving negotiation, adaptation, and
learning.

Within the South African context, policy implementation is also guided by constitutional
principles. Section 195 of the Constitution of the Republic of South Africa (1996)
requires public administration to promote accountability, transparency,
responsiveness, efficiency, public participation, and developmental governance.
These constitutional values imply that while national coordination remains necessary,
effective implementation equally depends on collaboration between government
institutions, public officials, and citizens.

1. Top-Down Policy Implementation

The top-down approach views policy implementation as a hierarchical administrative
process in which policy objectives are formulated by senior political leaders and
transmitted downward through government departments for execution. According to
Pressman and Wildavsky (1973), successful implementation depends upon clearly
articulated policy goals, effective administrative control, adequate resources, and strict
monitoring mechanisms. Under this model, senior policymakers retain authority over
decision-making while lower-level officials are expected to implement policies faithfully
according to predetermined rules and procedures.

Within the South African Department of Health, the top-down approach is evident in
the implementation of major national health policies such as the National Health
Insurance (NHI), the National HIV, TB and STI Strategic Plan, and the COVID-19
