Health History Overview
The information for this assessment was gathered using the Health History Questionnare.
The purpose of the Health History Questionnare is to collect subjective data, which comes
straight for the patients mouth, and combine it with objective data. The Health History
Questionnare is used to screen for abnormalities and health concerns or possible problems. Most
of the time, the patient fills out a printed form or completes the form electronically upon arrival
to a clinic or doctors visit.
Introduction
The Healthy History Questionnare covers 9 topics. The following topics are
demographics, perception of health, past health history, family health history, review of systems,
developmental considerations, cultural considerations, psychosocial considerations, and
collaborative resources. All of the topics listed above are answered in the perspective of the
patient.
Demographics
The patient that I completed my Health History questionnaire on was a 30 year old single
African American female. She had given birth to one daughter who is now 5 years old.
Reason for Care and Present Illness
Patient came into office for a well visit. Patient is showing no signs or symptoms of a
present illness.
Perception of Health
The patient stated during the interview that her perception of health involves the body,
mind, and spirit. She feels that when they all work together, that is how she reaches her optimal
