Scenario
You are working as an RN in a large women’s clinic. Y.L., a 24-year-old Asian woman, arrives
for her regularly scheduled obstetric appointment. She is in her 26th week of pregnancy and is a
primigravida. After examining the patient, the nurse-midwife tells you to schedule Y.L. for a
50-g glucose challenge. You review Y. L’s chart and note she is 5’3” and weighs 143 pounds, and
her prepregnancy body mass index (BMI) is 25. Her father has type 2 diabetes mellitus (DM),
and both paternal grandparents had type 2 DM. You enter the room to talk to Y.L.
1. You instruct Y.L. to drink a 50-g solution of glucose and have her blood drawn 1 hour
after ingesting the solution. What is the purpose of a 50-g glucose challenge?
A 50-g glucose challenge is not the only test done to predict gestational diabetes. This test does
not consist of fasting. The mother drinks the 50-g solution and after an hour, after she drank it
one blood sample is taken. There are two steps to this test. If the 50-g solution test results are
high then a 100-g solution test is taken next. This helps determine if a woman is at high risk for
gestational diabetes or already has gestational diabetes.
2. The results came back from the lab. Y. L’s blood glucose was 151 mg/dl. What does this
mean?
During pregnancy, a women’s glucose level should not be above a 140 mg/dl. If their blood
glucose exceeds 140 mg/dl then they are sent back in to get a 100-g solution test (3-hour glucose
tolerance test). For this test, the woman will have to fast 8-14 hours before the test is done. Then
the woman will drink the 100-g solution. The woman gets her blood drawn one hour before the
test and three times every 60 minutes after she drinks the solution.
