Client Name: Amir Patel
Situation: 89-year-old male admitted to the ED for altered mental status, fever, cough and
shortness of breath. Diagnosed with Pneumonia.

BP 104/68
Temp 100.1
O2 94% on RA
Background: The wife called EMS after she noted the patient becoming more lethargic and
displaying odd behaviors. Says he did not remember her this morning. He has not eaten or
drank much in the last 24 hours.
Assessments:
Vitals:
T: 100.2 oral. abnormal
BP: 86/56. Normal. Hypotensive.
P: 105. abnormal.
RR: 26. abnormal.
Sp02: 90% room air. 2L O2 via nasal cannula
Pain: 0/10.
Skin: Warm & dry. Tenting over clavicle after skin pinch test.
The patient is alert and oriented x2.
Medications:
Donepezil 10 mg PO QHS
Chlorthalidone 12.5 mg PO QD
Levaquin 500 mg IV Q12 hrs
Enozaprin 40 mg subQ QD
Normal saline 0.9% @ 75 mL/hr
Ipratropium bromide/albuterol sulfate per neb treatment Q4H
I&Os
O2 2L per nasal cannula
