This comprehensive i-Human case study is designed for advanced practice nursing students (Class 6541) and focuses on Leo Martinez, a 4-year-old male presenting with acute diarrhea and abdominal pain of 3 days' duration. The document follows the complete clinical reasoning process, guiding students through subjective data collection (HPI using OLD CARTS format), objective physical examination findings, differential diagnosis formulation (including bacterial gastroenteritis, viral gastroenteritis, post-infectious lactose intolerance, IBD, and appendicitis), diagnostic testing (stool PCR, culture, CBC, CMP), evidence-based treatment planning (oral rehydration, antipyretics, antibiotics, probiotics), and comprehensive patient/family education with specific return precautions . A key finding is a positive fecal occult blood test, which elevates concern for invasive bacterial gastroenteritis (e.g., Campylobacter, Salmonella, Shigella, E. coli O157:H7) . This case study includes the primary diagnosis of Bacterial Colitis (ICD-10: A09) and is aligned with APRN-level standards for pediatric gastrointestinal assessment . Updated for the 2025/2026 academic year, this resource is ideal for nurse practitioner students preparing for i-Human platform assessments, pediatric clinical rotations, or certification exams.
