Postpartum Assessment - Systematic postpartum assessment tool: Breasts
BUBBLLEE - check for engorgement, nipple trauma,
lactation issues. Uterus - fundal position,
firmness (involution), and location. Bladder -
voiding patterns; full bladder can displace
uterus and increase bleeding risk. Bowel -
assess for constipation, flatulence, bowel
movement. Lochia - type, color, amount, odor
(Rubra, Serosa, Alba). Legs - check for swelling,
DVT, mobility. Episiotomy/Laceration/Incision -
check perineum or cesarean incision using
REEDA. Emotions - assess mental status, mood,
fatigue, and bonding with infant.

Lochia Rubra (Days 1-3): Bright red, composed of
blood and decidual debris. Serosa (Days 4-10):
Pink or brown, contains serum, leukocytes, and
old blood. Alba (Days 10-14 or longer): White or
yellow, contains leukocytes, mucus, and
bacteria. Monitor for abnormalities: foul odor,
clots, or excessive bleeding.
