For this activity you will use EHR Go to complete hands-on assignments that replicate the
health care electronic health records experience for revenue cycle management.
In this exercise, you will review the data in the patient chart in detail to recognize areas of
deficiency in the medical record. Recognizing deficiencies is important for clinical
documentation improvements, query opportunities, and increase of reimbursement for
services potentially missed. There are two attempts for this activity. The highest grade will be
entered in the gradebook.

begin this activity.
3. Refer to the patient chart and any suggested resources to complete this activity.
4. Review the questions to extract answers from the EHR Go activity. You will need to
copy the questions or keep multiple windows active to answer the following graded
questions.
1) Review each area as indicated. Find the required field in the patient's medical record
for Jacy Sky Redbird. 2) Determine if the information is listed or deficient. 3) Select the
appropriate option for each question.
If the information listed in the question is present, this will be an example where any
further CDI or query is not necessary.
If the information listed in the question is deficient (lacking, sub-standard, or not
present), this will be an example of a good CDI or query opportunity.
For example:
Allergies
Allergies identified or "no known allergies" identified
A. Information is present - CORRECT
B. Information is deficient
*This field "allergies" or "No known allergies" is complete (contains the required
information) in the medical record for this patient.
The diagnosis, problem and/or reason for admission may be found on the Problems
tab.
