2. CASE INTRODUCTION
Brianna Gill is a 28-year-old Caucasian female presenting to an outpatient primary care clinic with a 2-3
day history of painful urination, urinary frequency, urgency, and suprapubic discomfort. She describes
the pain as a burning, sharp sensation "like glass" that occurs with urination and lingers for
approximately 5-10 minutes after voiding. She reports cloudy urine with a stronger odor and noticed
pink-tinged urine. She denies fever, chills, flank pain, nausea, or vomiting. She has no significant past
medical history aside from one prior UTI two years ago and is taking oral contraceptives. She is sexually
active with a new partner (approximately 3 weeks) and reports inconsistent condom use. She has a
penicillin allergy (rash). Initial urinalysis reveals positive leukocyte esterase, positive nitrites, WBCs 25-
50/hpf, and moderate bacteria, consistent with acute uncomplicated cystitis. Pregnancy test is negative.
Urine culture and STI screening are pending.

has no significant past medical history and is generally healthy. She is a nonsmoker, drinks 1-2 alcoholic
beverages per week socially, and denies illicit drug use. She is sexually active with one male partner of 6
months (new partner of approximately 3 weeks), with inconsistent condom use. She reports one prior
UTI approximately two years ago. She has a penicillin allergy manifesting as a rash. She currently takes
an oral contraceptive pill and a multivitamin daily.
