Module 3- Intrapartum Care
Labor Triggers
Maternal:
• Uterine muscle stretching
• Pressure on cervix
• Increased estrogen levels
• Decreased progesterone
Fetal:
• Prostaglandin synthesis
• Fetal cortisol levels increase
Fetal Assessment during labor :
• Electronic and Internal fetal monitoring External:
o Doppler
• Internal:
o IUPC: typically on the back of the baby
o FSC: Fetal scalp electrode: on the fetus head
Contraindications: HIV, infection, post GBS
Nursing intervention: For fetal heart tone need on a fat pt.
• Three-Tier System o FHR Patterns
o Uterine contraction pattern and
palpation o VEAL CHOP MINE
P - Position (typically left side)
O - Oxytocin (almost all cases it’s stopping the Med)
O - Oxygen (Admins Oxygen)
F - Fluid (giving fluids with decels)
Other interventions: Amnioinfusion, Terbutaline, Cervical Exam, decrease pain and anxiety, change tech.
Tachysystole:
- 6+ contraction in 10 min
- Contraction that last longer than 2 min
