Avery Brook is a 10-year-old female who presents with declining academic performance over
the past several months. Her mother reports that Avery was previously an average-to-above-
average student but has recently experienced a noticeable decrease in grades.
Mother reports:
• Difficulty maintaining attention during homework.
• Frequently forgets assignments.
• Easily distracted during class activities.
• Requires repeated instructions.
• Increased frustration when completing schoolwork.
• Teachers have reported incomplete assignments and poor concentration.
The patient denies headaches, visual disturbances, dizziness, seizures, or recent illness.
No recent trauma reported.
No significant changes in appetite.
Sleep averages 8–9 hours nightly, although mother reports occasional difficulty falling asleep.
