Abbreviated Standard SURVEY

,-Scope: limited to relevant areas of FA operation (unless survey
suggests others)
- Concerns that decline in QoC provided to nursing
home RESidents as result of certain changes (change in

EXTENDED Survey INCLUDES
* Conducted when a finding of SQC during a standard survey
includes following:
- review of a larger sample of RESident assessments
- review of staffing & in-service training
- surveyors to examine consultant contracts
- review of P&Ps related to noncompliance
- review of nursing services, physician services, administration, &
any other requirements
- Other requirements may also be reviewed at discretion of
survey team.
- A federal survey conducted within 14 days
EXTENDED Survey CONDUCTED as follows:
PRIOR to the exit conference (of the standard survey or revisit)
OR
, No later than 14 days after the completion of a standard
survey if the survey team is unable to conduct the extended
survey concurrently with the standard survey
The COMPREHENSIVE assessment must be

- The IDT lead by nursing will develop the CCP within 7 days.
- Prepared by IDT (attending MD, RN with responsibility for
resident, and other appropriate disciplines, and involvement of
resident or legal rep)
- IncludeS measurable objectives/timetables to meet resident's
medical, nursing, mental/ psychosocial needs
- Periodically reviewed/revised by team of qualified persons
after each assessment
