decreased bone mass (increased risk of fractures)

of treatment

Assessment:
Assess CBC with differential
Implementation:
Monitor temperature frequently. Assess for signs of infection including pain,
erythema, dysuria, mental status change
Assess status of immunizations to include pneumococcal vaccine,
meningococcal, seasonal influenza vaccine.
Administer filgrastim (Neupogen) as ordered. Assess for signs of bone pain.
Monitor CBC twice weekly to check leukocyte level.
Have the client remain in his room unless he needs to leave for a procedure
/ therapy. Place a mask on him during transport.
Protect the client from possible sources of infection (plants, change water
in equipment daily, restrict client to room).
Have client, staff, and visitors perform frequent hand hygiene. Restrict ill
visitors.
Avoid invasive procedures that could cause a break in tissue unless.
Keep designated equipment in the client's room (blood pressure machine,
thermometer). Private room
Nurses should not cohort neutropenic patients with infected patients
Instruct patient:
To avoid crowds while undergoing chemotherapy.
Avoid eating fresh fruits and vegetables
Avoid yard work, gardening, or changing a pet's litter box.
Avoid fluids that have been sitting at room temperature for greater than 1 hr.

Report fever > 100˚ F or other symptoms of infection
