1. INLEIDING
- Algemeen grote groei in wersterse populatie → sprake van Parkinson Pandemie
- Tremor = klacht die best te zien is → maar geen kern symptoom
- > → grotere predispositie
OPM: vaak verschillende klachten en uitingen
- Boxers → ook grotere predispositie → door klappen verschillende scheurtjes in de
hersenen → verworven microbloedingen thv basale kernen → geïnduceerde vasculaire
parkinsonisme
- Meestal idiopathisch
- ↑ kans bij ↑ leeftijd → maar ook kans bij jongeren en kinderen → vaker dan genetisch
bepaald

2. NEUROPATHOLGIE
Thv substantia nigra → zwarte stof = neuronen die dopamine produceren
 Deze neuronen gaan afsterven en degenereren
 Verdwijnen van de substantia nigra
 ZIEKTE VAN DE SUB-CORTICALE DEMENTIE (= basala ganglia en prefrontale cortex)

2.1. Motorische controle
2 grote banen
1) Habitueel → automatische baan
o Simpele situaties zonder specificiteit nodig
o = snel en efficiënt
 Door ↓ nood aan aandacht kan hierbij een dual task gebeuren
BIJ PARKINSON → hier aantasting want deze gebruikt de basale kernen
2) Doel gericht → aandacht baan
o Complexe situaties met hoge precisie
o Langzamer en vermoeiender
o ↑ afh. Van sensorische input
o Bij Parkinson hier geen problemen mee → deze gebruikt geen basale kernen

2.2. Pathofysiologie Parkinson
Het is GEEN UMN-aandoening → geen spasticiteit → Motor-neuronen zelf niet aangedaan,
enkel de aansturing ervan
 Als aansturing terug normaal zou worden → ook terug normaal bewegen

LEWY-BODY PATHO
Alfa-synucleine gaat aggregeren in een lewy-body → dit eiwit wordt dan niet bruikbaar
Lewy bodies gaan neuronen opvullen en verstoren zo metabolisme van neuronen →
neuron zal uiteindelijk afsterven
