Proposal
The proposed innovative, value-based healthcare change is the integration of artificial
intelligence (AI) into the electronic health record (EHR) system at a community hospital to
enable predictive analytics for patient risk assessment (Rose & Chen, 2024). This harmonization
will have a positive impact on patient care, as it will ensure early identification of at-risk patients
and enable timely interventions to prevent negative outcomes, such as readmission and
complications. The AI tool will review patient data in real-time, alerting healthcare providers to
risks such as sepsis or falls and promoting a culture change from reactive to proactive care
(Sarraf & Ghasempour, 2025).
In order to approve this proposed change, an organization sponsor who was the Nurse
Manager was recruited and met with for completion of the attached “Professional Verification
Form,” which confirms approval and details of the discussion. During the sponsor interview, we
discussed the feasibility of the proposal, how it would impact nursing workflows, and align with
the facility’s value-based care goals. The sponsor provided positive feedback, emphasizing the
need for staff training to facilitate adoption of the tool as well as recommending its integration
with existing EHR training programs. Although data privacy concerns were raised, the sponsor
was generally in favor of this initiative.
Some minor modifications were made to the implementation plan based on the sponsors’
input. For example, including an extra dedicated training module in the implementation timeline
to better address workflow integration. However, no major changes were required, as the
fundamental aspects of the proposal aligned closely with the input provided.
