1) How did the error occur? Was it a calculation error (math error or formula error)? Was it
a “silly” error (not checking or going to fast)?
This error occurred because I accidentally divided the patient’s weight by 2 instead of by 2.2. I
actually had the correct answer originally but I was double checking my calculations and
mistakenly used 2 instead of 2.2 when I redid the calculation and changed my answer.
2) Did you know the drug? Do you now? (hint…look it up if needed)
I was not familiar with this drug previously. Upon researching, I have learned that this drug is a
beta-1 adrenergic agonist. It is used in the treatment of cardiogenic shock and heart failure.
3) Consider if this error occurred in practice, describe how you would handle the issue?
I would immediately stop the infusion and do a quick head to toe assessment on my patient. I
would address any issues with airway, breathing and circulation and then I would let the
provider know of my mistake. I would then take the appropriate steps to document the error
and bring it to the attention of my nurse manager.
4) How would you monitor the patient if this error occurred?
As mentioned previously I would do a head to toe assessment and take a set of vital signs.
Because of the action of the medication, I would take special care to listen to their heart sounds
and monitor their heart rate. I would also check their radial and pedal pulses to assess for any
perfusion issues. I would also make sure to check their skin for any signs of rash or flushing.
5) How would you feel if you were involved in this error in clinical practice? Where would
you turn for support?
