3. History of Present Illness (HPI)
The patient is a 72-year-old male who presents with a 2-week history of
progressively worsening dyspnea. Initially, he noticed shortness of breath with
moderate exertion such as climbing stairs. Over the past several days, the dyspnea
has worsened and now occurs with minimal activity, including walking short
distances on level ground.
Dyspnea characteristics:
