, History of Present Illness (HPI)
A 33-year-old patient presents with flank pain that began [insert duration, e.g., “2 days ago”].
The pain is located in the [right/left] flank and is described as [sharp/dull/colicky] in nature. It
[radiates/does not radiate] to the [groin/abdomen/back]. The pain is rated [X/10] in severity
and has been [constant/intermittent].
The patient reports that the pain is [worsened by movement/relieved by rest or
medications/no relieving factors]. Associated symptoms include [nausea, vomiting, fever,
chills, dysuria, hematuria, urinary frequency/urgency]. The patient [denies/admits] recent
trauma, heavy lifting, or similar previous episodes.
There is [no/history of] kidney stones, urinary tract infections, or renal disease. The patient
has [not taken/has taken] medications for the pain, with [no relief/partial relief].
