gal status, income level, referral source. Includes whether to accept into the program or re-
fer elsewhere
Eligibility Criteria - ANSWER determined by the programs goals, target population, and
funding requirement. Criteria may include: sex, age, residence, legal status, veteran status
etc. Criteria related to the program may include environment of program and level of care

Referral - ANSWER Done if the client is not appropriate for companies services.
Intake - ANSWER The administrative and initial assessment procedures for admission to a
program. The decision to admit is formally made and documented.
Admission/intake form - ANSWER basic information: name, address, employer, family
composition, emergency contact etc
Initial assessment form - ANSWER brief statement about presenting problem and imme-
diate needs (detox, residential, outpatient etc)
