NAME OF MEDICATION, CLASSIFICATION, AND INCLUDE PROTOTYPE MEDICATION: - Metformin - Warfarin Sodium - Losartan - Clorthalidone CLASSIFICATION: - Metformin: antidiabetic, biguanide - Warfarin sodium: anticoagulant - Losartan: antihypertensive, ARB - Chlorothalidone: thiazide diuretic SAFE DOSE OR DOSE RANGE, SAFE ROUTE - Metformin: oral solution- 500mg/5mL, Tablet- 500mg-1,000mg. - Warfarin Sodium: PO tablets 1-10mg. - Losartan: PO tablets 50-100mg. - Clorthalidone: 12.5-100mg PO daily.- PURPOSE FOR TAKING THIS MEDICATION - Metformin: Diabetes-reduces blood glucose levels. - Warfarin Sodium: PE, DVT, MI and rheumatic heart disease, and atrial fibrillation- inhibits clotting factors. - Losartan: Hypertension- inhibits vasoconstriction, and increases fluid excretion by blocking angiotensin II receptors. - Clorthalidone: Hypertension- increases fluid excretion. PATIENT EDUCATION WHILE TAKING THIS MEDICATION - Metformin: Instruct the patent to stop taking this medication if hyperventilation, muscle weakness or dizziness occur. Educate patient on the importance of refraining from excessive alcohol intake. - Warfarin Sodium: Instruct the patient to be cautious with movement to prevent falls and induced bleeding. Monitor closely for signs of bleeding. Educate patient to refrain from diets high in potassium to avoid reducing the therapeutic effects of warfarin. - Losartan: Instruct the patient to avoid a high sodium diet, and to report swelling of the eyes, lips and face immediately. - Clorthalidone: Monitor blood pressure closely. Educate the patient on the risk for orthostatic hypotension and encourage patients to change positions slowly, discuss the risk for hypokalemia and encourage a high potassium diet.
