Chief Complaint (CC)
"My skin has been really itchy for the past week, especially on my arms and legs."
• Duration: 7 days
• Location: Forearms, legs, and occasional patches on the torso
• Character: Persistent, pruritic sensation with small raised bumps
History of Present Illness (HPI)
13-year-old female who presents with complaints of generalized itchy skin for the past 7 days,
primarily on her forearms, legs, and occasional patches on the torso. Her mother reports that
the itching began gradually and has been progressively worsening. Emily describes the itch as
moderate in intensity (6/10 on subjective scale), persistent throughout the day, and worse at
night, which interferes with sleep.
The mother denies any recent infections, fever, or systemic symptoms. Emily reports occasional
small red bumps on affected areas but denies blistering, oozing, or pain. She notes that
scratching temporarily relieves the itch, but it often leads to increased redness and irritation.
Cool baths and moisturizing provide temporary relief.
Emily’s mother states that Emily has never experienced a similar rash before, and there is no
known exposure to new soaps, detergents, or environmental allergens in the past week. She
recently returned from a week-long camping trip, during which she wore shorts and t-shirts
most of the time. Emily denies recent contact with anyone with a similar rash.
The patient has no history of asthma, allergic rhinitis, or eczema, but her maternal aunt has a
history of eczema. There is no history of recent medication changes, insect bites, or new foods,
and she has not applied any new lotions or creams recently.
