Project Team Members
I meticulously select my project team members based on their skills and knowledge of the clinic
flow. These members contributed their expertise during the project's planning and
implementation phases. I chose Rashonta, a medical assistant, to be a project team member. She
was selected because she is an independent worker and she is well-liked by the clinic staff.
Rashonta is responsible for making phone calls to patients to remind them of their upcoming
appointments. She is responsible for combing through charts to verify which patients confirmed
their upcoming appointments in the electronic medical record weekly. This process usually takes
about twenty minutes to search the schedule and complete reminder calls to patients, and the
entire process can be completed within thirty to forty-five minutes. Patients who did not confirm
their appointment and are unable to attend will have their appointments rescheduled by
Rashonta.
Michelle is an LPN, and she makes the assignment for the call list, which is distributed to the
clinic staff. She gives the clinic staff the resources to decide which patients will be a priority for
reminder calls. The medical assistants report to Michelle. Charnel is the Lead Patient Access
Representative. She will aid with calling patients and scheduling appointments for patients who
require overbooking. She will be working closely with all the members on the health
improvement project.

The Clinical Manager, Amarachi O., serves as the Clinic Manager and the Educator for the HIP.
One role Amarachi will have is a template builder for the providers' schedules. For this role, she
is responsible for building scheduling templates for the clinic's providers. This task will allow for
accommodations to change in the clinic schedules. Template changes will also allow for
accommodations that will consider the patient's and the provider's availability. Amarachi can add
and block slots to a clinical session. A second role for Amarachi will be as the project educator.
Her responsibilities for this role include educating the project team on the data from the clinic
revenue report and the no-show reports. As the project manager, I am accountable for the
comprehensive oversight of the project. I am also responsible for defining project goals and
objectives, assigning tasks and delegating responsibilities to stakeholders, and coordinating the
resources necessary to sustain both project operations and team performance. Additionally, the
project manager provides training, disseminates educational materials, and evaluates project
outcomes. All team members meet once weekly for 30 minutes to determine which needs are met
and which tasks need revision.
Justification for the Project Team
The justification for selecting the members of the project team is that they have diverse skill sets
and are the most proficient at their jobs. My project team has specialized skills that are needed to
carry out the healthcare improvement project by increasing patient compliance with their
scheduled appointments. Michelle is one of our top LPNs in our clinic. She works well with the
clinic staff and the ancillary staff. Michelle has been the longest-serving LPN at the clinic. She
knows the needs of the providers that we have. Michelle understands and knows how the clinic
flows. She has the resources, skills, and talent to make this healthcare improvement project
successful. Rashonta is a medical assistant at the clinic. She is a proficient medical assistant. She
