Introduction
The National Institutes of Health Stroke Scale (NIHSS) is a standardized tool used
to quantify neurological deficits in patients with acute stroke. It evaluates several
domains including level of consciousness, language, motor function, sensory
perception, gaze, visual fields, ataxia, and attention.
This dataset represents 36 patients across six groups (A–F), with each patient scored
on the 11 NIHSS items. Scores are presented in a line-by-line format, and all deficits
are bolded for clarity. This format is intended for study, clinical reference, or
training purposes in acute stroke assessment.
Purpose: This document provides the complete NIHSS scoring for 36 patients
across Groups A–F, highlighting neurologic deficits for rapid assessment.
Scoring Key:
0 = Normal / No deficit
1–4 = Minor deficit
5–15 = Moderate deficit
16–20 = Moderate to severe deficit
21–42 = Severe deficit
