prescriber or other licensed HCP
3. Provision of advice or consultation with the
patient
4. Dispensing drug information to the patient
5. Receipt of verbal prescription other than a refill
approval or denial from a prescriber
6. Final check on all aspects of the completed
prescriptions and assumption of responsibility for
the filled prescription.
Immunizations in Indiana Must have board approved training in the giving of
vaccination
Must notify physician who authorized the
immunizations and the individuals PCP that the
individual received the immunization within 14 days
of administration
>11 years of age
>50 years of age if receiving the pneumonia

presents evidence
2 Practioner allowed legal representation
3 If sanction imposed allowed to appeal in court of

Sanctions: Revocation Permanent license takeaway which you cannot
practice.
Cannot be reinstated.
