HISTORY OF PRESENT ILLNESS (HPI)
The patient is a 16-year-old male who presents with hand pain. The pain is localized to the hand
and began after routine activities. He denies any major trauma. The pain is worsened with
movement and improves with rest. He denies numbness, tingling, or weakness. There is no
associated fever or systemic symptoms. The pain prompted evaluation in the outpatient clinic.
REVIEW OF SYSTEMS (ROS)
General: Denies fever, chills, weight loss
HEENT: Denies headache or vision changes
Cardiovascular: Denies chest pain or palpitations
Respiratory: Denies shortness of breath
Gastrointestinal: Denies nausea, vomiting, abdominal pain
Genitourinary: Denies urinary symptoms
Musculoskeletal: Positive for hand pain
Neurological: Denies numbness, tingling, or weakness
