History of Present Illness (HPI)
The patient is a 16-year-old adolescent who presents with acute
right hand pain that began 24 hours ago. The pain started after
falling onto an outstretched hand while playing sports. The
patient describes the pain as sharp and throbbing, rated 7/10,
worsened by movement and gripping objects, and mildly relieved
with rest.
The patient reports:
Swelling around the knuckles
Decreased range of motion
Mild bruising
Pain with finger flexion
Denies:
Numbness or tingling
