“Back pain.”

History of Present Illness (HPI):

Basilia Barco is a 53-year-old white female who presents with lower back pain that began
approximately 2 weeks ago. The patient describes the pain as a dull, aching sensation located in
the lumbar region, rated 6/10, with intermittent sharp exacerbations during movement.

The pain began after lifting a heavy box at home. She reports the pain worsens with prolonged
standing, bending, and twisting, and improves slightly with rest and over-the-counter ibuprofen.
She denies radiation of pain to the legs, numbness, tingling, bowel or bladder dysfunction, or
saddle anesthesia.

She denies fever, chills, recent trauma, unexplained weight loss, or history of cancer. No previous
episodes of similar severity.
