Micro Level.
1. Health Literacy & Empowerment Programs.
The churches and community centers will receive custom chronic disease
prevention and selƒ-management workshops. Through these workshops
conducted by trained community health workers (CHWs) and with support
ƒrom nurses and doctors, participants will learn about heart attack, diabetes,
and other diseases.
2. Patient Navigation Support.
Ƒor this study, we will use local Aƒrican American patient navigators trained in
cultural competency to help patients obtain care, learn about treatment options,
and complete their appointments to build trust and enhance continuity oƒ care.
Mezzo Level.
1. Mobile Community Health Units.
The mobile units will go to underserved neighborhoods ƒor basic screenings, care,
and behavioral health services. They will coordinate with local shelters and
addiction recovery centers to serve people who experience homelessness or
addiction disorders.
2. Ƒaith and School-Based Partnerships.
We could work with churches and schools to bring services to ƒamiliar places in
the community. More ƒamilies and youth will connect with us! School nurses and
chaplains will host wellness days on maternal health, nutrition and early
screening.
