P4 Plan a care pathway for both disorders
In this task I would be planning a care pathway for two physiological disorders which are
Alzheimer’s and Diabetes.
What is a care pathway ? Care pathways can be described as the way in which
professional services are brought together in order to meet the needs of the individual over a
period of time .
The purpose of a care pathways
● To ensure consistent standards of care in the same department .
● To ensure that everyone in care including the patient would know what they are
receiving .
● it identifies cost and resources .
● It also makes it easy to know the cost and resources of a patient .
● It empowers the patient and staff .
● It also eliminates unnecessary and repetitive information that is not needed .
Primary care : this is the type of care that occurs in the community outside hospitals , this
includes GP’s dentists,opticians and pharmacists.
Secondary care : This is a test carried out in general hospitals usually after a referral from
the GP.
Tertiary care : These are specialist care establishments such as stroke rehabilitation clinics,
spinal injury unit and cancer care (oncology) units,

Care pathway
This is also known as clinical pathways and critical pathways. It's regarded as one of the
best tools hospitals can use to manage the quality in health care.
Care pathway for Alzheimer's disease.
Alzheimer's disease is a progressive disease that destroys the memory and other important
mental functions. There is no cure in existence for Alzheimer's disease but medications and
a good management strategy can temporarily improve the symptoms.
Requirement for planning a care pathway (ALZHEIMER'S)
There are three important principles that any dementia care pathway should be based on;
● Raising awareness and understanding of dementia.
● Early diagnosis and support for those living with it and their families.
● Making patients and their loved ones able and empowered to live well with dementia
and taking care of a loved one with dementia .
Alzheimer's care pathway describes the care that individual is given from the moment the
patient is being diagnosed with Alzheimer's till the day they die.

Alzheimer's affects people in different ways at each stage of the condition e.g. severe , mild
and moderate .
Care pathway for coronary heart disease
Coronary heart disease just like Alzheimer's disease can't be cured but with treatment you
can reduce the risk and manage the symptoms thereby reducing the risk of a heart attack.
An example of a care pathway for Coronary heart disease includes
● Changing one's lifestyle exercising more and getting rid of smoking
● A care plan that clearly indicates Intake of medicine as prescribed .
● Eating healthy and maintaining a good balanced diet .
● Controlling blood cholesterol and sugar levels regularly.
● Risk assessment of cardiovascular diseases regularly.
● Arrhythmias/palpitations: This is regular diagnosis and management of atrial and
ventricular arrhythmias which are mostly done in secondary and tertiary care setting .
