Red eye
History & symptoms:
● Introduction
● Address RFV: “so I can see that you have come in today because of”
● “ Could you please explain what has happened?”
● Location: eye? Is the redness in one particular section or is it all over on the white of the eye?
● Onset: when did it start? sudden/long-standing?
● Frequency: is it constantly red or does the redness come & go?
● Type: how red is it- bright, blue-red? Eye pain (out of 10, on the eye, behind the eye, pain on eye movement)/
ache/itchiness/burning/gritty sensation;
discharge/ light sensitivity / blurry vision?
● Self-treatment: does anything make it better/ worse?
(eye drops, cold compresses, warm compresses, breaks from screens, wearing the lenses)
● Associated sxs

● Has this happened before?
● Any changes in the environment?/ Were you doing anything when this had happened?
● Other questions
Headaches? (Any triggers/patterns?)
Diplopia?
● GH: how is your general health like? Have you been ill recently? Any diagnosed medical conditions or conditions
where you are taking medicines for? Any allergies like hay fever/seasonal/all year round? Any asthma/ eczema? Any
inflammatory/ autoimmune/ connective issue? Smoker?
● Meds: meds/drops/ ointments
Antidepressants, Acne med, contraceptive pills, glaucoma drops, topical preservative drops
● OH: CLs: Type/Name? If reusable, any solutions or has it been changed recently? How often do you wear CLs?
When was the last AC/what did they find or did they advise you on anything? Do you sleep,swim, shower? Does
discomfort/pain worsen when removing lenses?
Any HES/ infections/injuries/surgeries?
● FH: autoimmune, inflammatory, connective tissue condition?,Glaucoma?
● Lifestyle: VDU, job, hobbies?
