HPI: W.C., 22yo female, comes into the clinic today complaining of vaginal
itching and discharge. Discharge is white, thick, minimal amount, no smell.
Both itching and discharge started about 5 days ago after using a new bath
bomb husband bought for pt. Does not have a history of issues with bath
products but this was different kind than usual. Describes the itching and
discharge as embarrassing and bothersome. Tried OTC miconazole with no
relief. Symptoms have been persistent since starting. No dysuria, hematuria,
or difficulty with urination. Patient reports no issues with menstrual cycles,
last period started 3 weeks ago. Patient does not use condoms, is in
monogamous relationship with husband. Negative pap smear and STD
testing 2 years ago. Denies fever, fatigue, abdominal pain, shortness of
breath, and chest pain.
