A1: Moving to a value-based model:

The majority of the population that is served in McCracken County by the Paducah

Health Medical System (PHMS) lives in rural areas and has a higher rate of diabetes,

hypertension, and heart disease when compared to the rest of Kentucky (Western Governors

University, n.d.). PHMS has seen a higher readmission rate over the past three years, as well as

overuse of the emergency department compared to other organizations in the state. Inadequate

patient outreach and low health education have been cited as root causes of these issues.

Adopting a value-based model of care will not only enable PHMS to better manage the

health of its rural communities to address these issues but also bring financial benefits. This

model financially rewards organizations for the quality of care provided, encouraging proactive

and preventive care over a reactive approach (Performance Health Partners, 2019). With a

technology upgrade, the value-based model will be further bolstered. This upgrade will support

PHMS’s efforts to improve patient outcomes by enabling robust data analytics for population

identification, care gap identification, stratification, and care management. Costs will be reduced

by improving the patient’s experience, risk will be reduced, reimbursements will increase, and

patients will be more actively engaged in their own healthcare management, all of which support

PHMS’s goals for improving its population’s health.



A2: Current state of information technology:

The current state of information technology at PHMS is a system called HealthData that

was customized for the organization nearly thirty years ago. It is not compatible with external

systems, including Lourdes Hospital, nor is there interoperability between its own departments

(Western Governors University, n.d.). Over the years, the server has become fractured, with
