Background Statement

Chester Hill Regional is a large urban hospital that is experiencing a dramatic increase in their
inflow of patients, due to the shut-down of another local facility. In addition to the increased
volume of patients, the hospital is currently short-staffed within their emergency department.
One evening, a 47-year old patient, Mary, came into the ED with abdominal pain. Dr. Dennis,
the physician on shift immediately ordered pain medicine and fluids. Due to the volume of
patients, Mary was moved into an overflow room. Several hours later, Mary was found in cardiac
arrest. Despite the efforts of the emergency staff, Mary was unable to be resuscitated. Upon
investigation of the matter, the cause of Mary’s death was due to her blood-alcohol content
mixed with hydromorphone. The intended focus of this case is teamwork and communication.

Major Problems and Secondary Issues

Lack of Teamwork within the emergency department increases the likelihood of medical errors
from occurring. Emergency department staff not working together can directly impact patient
care.
Lack of communication between the physician and nursing staff contributed to the death of the
patient. The patient was not monitored closely, which resulted in death.
Short Staffing contributes to the problem and needs to be addressed to prevent future errors.
Lack of a Focus on Quality is also a contributor to the situation.

Your Role

Chief Medical Officer (CMO) of the hospital is the best role. This person can leverage various
departments to develop a solution. Potential challenges to addressing the case from this role
include the adverse response from staff members of the ED, due to the current staffing issues.
This can delay any progress being made, in addition it may lead to employees being
uncooperative.

Organizational Strengths and Weaknesses

Strength: Chester River Regional Hospital is a large hospital with the ability to serve a sizable
portion of the area’s population. In addition, the hospital will see an even greater demand for
care than ever before, due to the recent shut down of a competing hospital.
Weakness: Despite the increased demand of patients, hospital employees have not been given
additional help or resources. It becomes increasingly difficult for nurses and physicians to
provide the same quality of care to patients when they are overworked and pulled in several
different directions at once.

Solutions and Recommendations

, For teamwork: The CMO works directly with HR to implement team effectiveness workshops.
These workshops provide employees with an understanding of their personal behavioral style, as
well as ways to work and interact effectively with their coworkers.
For communication: The implementation of the SBAR communication tool. This tool is a
communication tool used for communicating critical information that requires immediate
attention and action. (SBAR: Situation, Background, Assessment and Recommendation).
For communication: Implementation of communication training workshops. Participation in
these workshops will be required by every staff member. This will provide staff with effective
communication tools and problem-solving skills that can be used in the workplace.
For dealing with the staffing issue: the ED director needs to make the case for additional staff
to address increased demand for care.
For improving quality: a quality improvement project might be undertaken to assess what went
wrong, determine how to prevent future occurrences, and use this as a teachable moment with
staff.

Evaluation

For communication and teamwork: students should cite research that demonstrates the
correlation that effective teamwork and communication has on medical errors. Then quantify
with, “the number of adverse events and medical errors will decrease by X within Y months”
