1.Lapsed License: A licensee who fails to renew license in a timeley
manner
2.Group supervision: The process of clinical supervision of more than one
person but no more than 6 persons in a group setting provided by an
LPC-S
3.The application process for licensure must be completed within days
of the board's receipt of the applicants package: 365
4. semester hours or quarterly hours of graduate study is re-
quired: 60,90
5.A maximum of hours of direct service may be obtained per work
week: 25
6. hours of individual face to face supervision are required: 100
7. hours of group supervision is equivalent to hour of individual
supervision: 2,1
8.synchronous video: Distance or telehealth supervision sessions
provided by secure means
9.A candidate must take and pass the required LPC examination by the
year mark of the receipt date of application: 4
10.Upon a final decision by the board not to license, the applicant must wait a
period of year before resubmitting a new application: 1
11.An LPC who fails to pay the required biennial renewal fees by the due date will
be considered in beginning immediately: Lapsed status
12.Active status: A licensee who is in good standing
13.Restricted status: The board, as a result of a disciplinary action, may
choose to limit or restrict the scope of a licensee's practice
14.Revoked status: The board may revoke an individuals license as a
result of disciplinary action
15.Suspended status: The board, as a result of a disciplinary action, may
choose to invalidate a license for a period of time
16.Surrendered status: A licensee, as a result of disciplinary action, may
request surrendered status
17.The P-LPC shall be renewable for not more than__________years: 4

, 18.Each P-LPC must accrue CEH hours and of which must involve
topics in professional ethics or legal issues: 6,2
19.The P-LPC license status will be changed to for all P-LPC's who fail

status must pay a reapplication fee in addition to the current year's renewal fee
